Prior to your first appointment, it is recommended that you have your records from your previous dental office, including radiographs forwarded to the office for review by Dr. Oshetski. This will help us prepare in advance for your first appointment.

Your First Visit
What should I expect at my first visit?
Your first visit to become an established patient of record will consist of a complete evaluation of your health history, a thorough examination by Dr. Oshetski, any necessary radiographs, and a comprehensive review of your past dental history. At any time you are encouraged to ask any questions that you might have about your particular dental situation and about our practice.
Patient Forms
Please download and complete our New Patient Forms. After you have completed them, please make sure to bring them on your first visit to our office. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.
